Patient Coordinator

We are seeking a dedicated and compassionate individual to join our healthcare team as a Patient Coordinator. The Patient Coordinator will play a crucial role in ensuring a seamless and positive experience for patients throughout their healthcare journey at Lift Cancer Care Services. This position requires excellent communication skills, organisational abilities, and a commitment to providing outstanding patient care.

QUALIFICATIONS & ATTRIBUTES
High school diploma or equivalent
Previous experience in a healthcare or customer service role is desirable.
Previous experience using clinical software ‘Direct Control’ an advantage
Strong organisational skills and attention to detail.
Excellent communication and interpersonal skills.
Knowledge of medical terminology and basic understanding of healthcare processes.
Ability to handle sensitive information with confidentiality.
Proficient in using computer systems and electronic health records.

Key Responsibilities
Patient Scheduling:
Coordinate and schedule patient appointments, ensuring efficient use of clinic resources.
Be the primary point of contact for all new referrals into Lift Cancer Care Services
Work closely with healthcare providers to optimise appointment availability and accommodate patient preferences.
Handle rescheduling and cancellations with sensitivity and efficiency.
Patient Communication:
Serve as the primary point of contact for all new patients, addressing inquiries, providing information, and offering assistance.
Communicate effectively with patients regarding appointment reminders, pre-visit instructions, and any necessary follow-up.
Facilitate patient accessibility to Lift clinical services by expertly managing patient enquiries with regard to clinic time changes and appointment changes
Intake and Registration:
Collect, process and verify patient information including insurance details, medical reports from the patient’s broader treating team, and new patient intake forms during the registration process.
Ensure accurate and up-to-date patient records, adhering to privacy regulations.
Patient Advocacy:
Advocate for patients by addressing concerns, facilitating communication with healthcare providers, and ensuring a patient-centric approach to care.
Collaboration with Healthcare Team:
Work closely with the Lift Cancer Care Services healthcare professionals to coordinate patient care effectively.
Communicate relevant patient information to the healthcare team to enhance the quality of care.
